– Outpatient Rehabilitation II;
– Supportive Rehabilitation.

• diseases of the nervous system;
• musculoskeletal disorders;
• diseases of the circulatory system;
• gynaecological diseases;
• kidney disease;
• diseases of the digestive system;
• endocrinological diseases;
• ear, nose and throat diseases;
• respiratory diseases;
• exposed to ionising radiation.

 

Documents required for entry for rehabilitation treatment:

• electronic referral for rehabilitation services;
• a referral for rehabilitation services is issued by the attending physician on the recommendation of a Physical Medicine and Rehabilitation (PM&R) doctor, or the PM&R doctor completes the E027 form in ESPBI IS. The dispatch is valid for 2 months. or up to 180 days if your doctor decides otherwise;
• electronic or printed extract from medical records 027/a (inpatient epicrisis);
• proof of identity.
